Who we are looking for: Experienced LCSW passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery. Provides therapeutic interventions and resource recommendations that address patient goals to achieve their personal, career, and independent living objectives in an integrated setting. Responsibilities What you will do (responsibilities listed are included but not limited to): Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families. Sets measurable objectives formulated in conjunction with the rehab team. Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems. Instructs/counsels patients and families in treating and coping with social and emotional responses connected with illnesses. Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs. Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Qualifications Master’s Degree in Clinical Social Work (LCSW). Current Licensed Clinical Social Worker licensure in the state of practice. Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis. Current BLS (Basic Life Support) First Aid and CPR certification required. Ability to lift 50 pounds. Capability to lift up to 100 pounds following safety procedures during moving, lifting, or transferring of patients. Fine motor skills, visual acuity, and ability to walk/stand for extended periods. Physical capabilities including pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently. #J-18808-Ljbffr
