PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.Responsibilities:Act as a primary case manager to 3-6 consumersProvide nursing interventions to individualsFacilitate and coordinate medical care to include communication with health care providersProvide health education to individualsAssist in the management of medications and the overall functioning of the nursing departmentFormulate and regularly update Comprehensive Assessments and IAP's for clients on your caseloadTransport consumers to work sites, meetings, daily activities, appointments, and activities as needed.Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clientsQualifications:Requires an associates degree in nursingLPN or RN nursing licenseExperience working with adults with mental illness and knowledge of psychiatric and medical medicationsValid Massachusetts drivers licenseSchedule: Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall scheduleBilingual candidates fluent in a needed language may be eligible for a 6% salary increase Annual Salary $58,073.60—$77,680 USD At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
