Nurse Case Manager , you will support youth with emotional, psychological, and physical needs within a residential setting. Role Overview: As a Nurse Case Manager , you will support youth with emotional, psychological, and physical needs within a residential setting. Key Responsibilities: Provide medical case management, screening, education, counseling, and medication management Network with community healthcare providers Participate in a rotating on-call schedule, primarily offering remote phone consultations to ensure accessibility Qualifications & Skills: BSN or ASN from an accredited nursing program Current Illinois Registered Nurse licensure and Driver’s License Experience with psychiatric care and children/adolescents is preferred Strong organizational, communication, and clinical skills Additional Information: The role offers flexible scheduling, benefits including health insurance, retirement plans, paid leave, and professional resources for loan forgiveness navigation.
