Service Coordinator for Home Care

- Oversee the intake process for all new clients, ensuring timely and accurate eligibility verification and service set up.
- Receive and review referrals for new clients, verifying eligibility for waiver services and confirming active service authorizations with the relevant county.
- Conduct intake assessments to gather comprehensive information about the client’s care needs, medical history preferences and support network.
- Coordinate with the family, county coordinator and medical providers to complete all required intake documentation within the timeframe specified by DODD, Medicaid, Medicare, Private Insurance.
- Ensure all intake documentation is filed accurately in the client's record prior to the first service delivery date.
- Attend county-level team meetings or care conferences for assigned clients as required.
- Serve as the primary point of escalation for any service delivery issues, disputes or authorization delays affecting clients.
- Follow all Standard Operating Procedures pertaining to Beulah Home Care LLC and comply with all applicable federal and Ohio state regulations.
