Our flagship transitional work program-- Ready, Willing & Able-- integrates paid work, occupational training, and holistic supportive services to facilitate an individual's return to mainstream society.We are seeking a Mental Health Counselor for the Crotona Park residence which is The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. The majority of Crotona Park residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and some by the New York City Housing Authority (NYCHA).The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.We offer a competitive salary with comprehensive benefits including medical and dental coverage, paid participation in a retirement plan, sick and vacation leave, discount gym memberships, paid holidays, and more.Key Responsibilities:Maintain a caseload and provide regular case management services to these individualsMaintain resident records in compliance with agency and funding standardsConduct and review assessmentsDevelop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiencyMonitor and ensure residents are linked to healthcare providersProvide counseling and crisis intervention as necessaryAssist residents in securing and maintaining benefitsMaintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programsFacilitate groups and activities that promote health and independent living skills developmentMonitor and assist residents with rent payments and budgetingConduct regular unit inspectionsEducate residents about approaches to mental healthCompile resources on mental health information and treatment strategiesAttend ongoing training to enhance the services provided to the residentsAssist with completion of HASA's weekly and monthly reports as well as any other relevant reportsOther duties as assignedThe Ideal Candidate:You have a Master's Degree in Social Work or a related fieldYou have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illnessYou have at least 5 years of experience in the mental health fieldYou have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issuesYou have solid computer skills – internet, Microsoft Office, and charting databaseYou have strong organizational, writing, and communication skillsIt's a bonus if:You are bilingual (English and Spanish)Interested?
