Hospice RN Case Manager

Responsibilities Completes initial assessment of patient and family to ascertain home care needs Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group Provides direct nursing care to patients in order to maintain highest level of care for the patient in conjunction with the primary physician Plans and supervises the delivery of care by the HHA Teaches family members, facility staff, hired caregivers, patient care skills Attends care plan meetings and/or patient care rounds for hospice patients within the facility Requirements Current Maryland State license as a Registered Nurse Diploma, ADN and/or BSN from an accredited institution Two years of nursing experience, or equivalent combination or education and experience Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection Strong time management skills Skill in the use of EMR (Electronic Medical Record) and/or ability to learn Hard Skills nursing care care plan implementation patient assessment infection control time management patient care skills supervision of care delivery collaboration with interdisciplinary team hospice care EMR usage Soft Skills communication teaching leadership organization collaboration Certifications & Qualifications Registered Nurse license ADN BSN #J-18808-Ljbffr Jobtailor
