Occupational Therapist Home Health Rehabilitation
Location: San Ramon, CA 94583 Duration: 13 Weeks Shift: 58-Hour Days Hours: 36-40 Hours per Week Guaranteed Hours: Dependent on facility requirements
Start Date: ASAP (Compliance takes approximately 2-3 weeks)
Coverage Areas: Area #11: Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos Area #12: San Jose
Job Details:
- Home Health Occupational Therapist position
- 1:1 patient ratio
- Up to 5-6 patient visits per day
- 27 productivity points standard
- Guaranteed 36 hours weekly after ramp period
- No on-call required
- Local candidates accepted
- Weekend visits optional to meet productivity
Requirements:
- Minimum 1 year of Home Health experience required
- Active California OT License required
- BLS Certification required
- First-time travelers not accepted
- Must have own transportation

- Mileage reimbursement provided at IRS standard rate

- Documentation systems: MediScrib & OASIS (training provided)
