You will work closely with RNs, physicians, nursing leadership, CNAs, and other interdisciplinary team members while exercising clinical judgment within your scope of practice.This position is ideal for a nurse who values hands-on resident care, professional accountability, teamwork, and the opportunity to make a meaningful difference during every shift.Key ResponsibilitiesProvide direct nursing care in accordance with individualized care plans, physician orders, professional standards, and facility policiesMonitor residents throughout the night and promptly recognize changes in physical, cognitive, behavioral, or psychosocial conditionReport significant changes in condition to the appropriate RN, physician, nursing leadership, and responsible party in accordance with facility protocolsAdminister medications, treatments, and therapeutic interventions safely and accurately within the LPN scope of practiceComplete scheduled treatments and ensure resident clinical needs are addressed throughout the assigned shiftObtain and monitor vital signs, laboratory results, treatment outcomes, and other clinical information as appropriateRespond promptly to resident needs, urgent situations, and changes in condition using sound nursing judgmentAssist with admissions, readmissions, transfers, and discharges as assignedCommunicate effectively during shift-to-shift report to promote continuity and accuracy of resident careCollaborate with RNs, physicians, pharmacy, therapy, dietary, social services, and other interdisciplinary team members to coordinate resident careStaff Supervision & LeadershipProvide direction, delegation, and support to CNAs and other nursing support personnel during the assigned shiftEnsure resident assignments are completed appropriately and in accordance with individualized care plansMonitor the quality and timeliness of care provided by nursing assistantsProvide real-time coaching, education, and clinical guidance to nursing support staffPromote accountability, teamwork, respectful communication, and professional conduct throughout the shiftReport staffing, performance, resident care, or safety concerns to nursing leadership promptlyAssist with maintaining appropriate floor coverage and resident care during changing staffing needsDocumentation & Regulatory ComplianceMaintain accurate, complete, and timely documentation within the electronic medical record (EMR)Document medication administration, treatments, assessments, changes in condition, physician notifications, and resident responses appropriatelyEnsure physician orders and clinical recommendations are implemented and followed through as assignedMaintain compliance with applicable CMS requirements, Kansas regulations, facility policies, and professional nursing standardsProtect resident privacy and confidentiality in accordance with HIPAA and facility requirementsParticipate in facility survey readiness and maintain compliant nursing practices throughout each shiftQuality, Infection Prevention & Resident SafetyFollow infection prevention practices, including hand hygiene, appropriate PPE use, isolation precautions, and transmission-based precautionsParticipate in quality assurance and performance improvement initiativesMonitor for and promptly report resident safety concerns, falls, skin concerns, medication issues, infections, changes in condition, and other clinical risksFollow established procedures for incidents, accidents, allegations, and other reportable eventsPromote a safe, clean, respectful, and resident-centered clinical environmentSupport facility initiatives designed to improve resident outcomes and reduce preventable adverse eventsQualificationsGraduate of an accredited practical nursing programCurrent, active Licensed Practical Nurse (LPN) license in good standing in the State of KansasCurrent CPR or Basic Life Support (BLS) certificationSkilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferredStrong clinical observation, assessment, and critical-thinking abilities within the LPN scope of practiceAbility to recognize changes in resident condition and escalate concerns appropriatelyDemonstrated ability to delegate, supervise, and support nursing assistantsExcellent communication and interpersonal skillsStrong organizational
