Job Title: Licensed Practical Nurse Travel (LPN)

Location: Vestal, NY

Assignment Duration: 16 Weeks

• Shift: Evenings/Nights
• Hours: 8-hour shifts
• Schedule: 5 shifts per week
• Weekend Requirement: Every other weekend
• Shift Changes: Shift assignment cannot be changed once offered. Requests for 12-hour shifts are subject to staffing approval.

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The ideal candidate will have experience caring for geriatric residents, including individuals with Alzheimer's disease and dementia, while demonstrating strong clinical skills, professionalism, and a commitment to patient-centered care.

• Administer medications and treatments safely and accurately according to physician orders.
• Perform medication passes while adhering to facility policies and medication safety standards.
• Assess residents and monitor for changes in condition, reporting significant findings to the healthcare team.
• Provide direct nursing care to residents in accordance with established care plans.
• Complete accurate and timely clinical documentation in the medical record.
• Supervise and support Certified Nursing Assistants (CNAs) on assigned units.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure quality resident care.
• Maintain infection prevention and patient safety standards.
• Promote resident comfort, dignity, and quality of life through compassionate care.
• Perform additional nursing duties as assigned within the scope of practice.

Education

• Graduate of an accredited Licensed Practical Nursing (LPN) program.

Experience

• Minimum 1 year of Long-Term Care (LTC) experience required.
• Candidates with recent experience in rehabilitation, medical-surgical, or hospital settings will also be considered.
• Experience caring for residents with Alzheimer's disease and dementia is preferred.

• Active New York State Licensed Practical Nurse (LPN) License .
• Basic Life Support ( BLS ) certification through the American Heart Association (AHA) .
• Proof of active New York LPN license must be provided at the time of submission.
