Position Details

We are seeking a motivated Occupational Therapist (OT) for a Home Health position. This role requires strong clinical skills to support adult and geriatric patients in the community setting.

Shift Information

• Shift Type: Day shift
• Schedule: 5 days a week, 8-hour shifts (08:00 AM - 04:30 PM)
• Guaranteed Hours: 40 hours per week
• On-call requirements: None
• Weekend coverage: Not required

Certification and Requirements

• MOAB Certification required. If not held, a 4-hour de-escalation course will be provided in the first week.
• National certifications required: MOAB, NBCOT, and NM license.
• BLS certification is necessary.
• Home Health travelers (RN, OT & PT) must complete a second set of fingerprints upon arrival for CYFD if children are involved.
• COVID vaccination is optional, but reporting of COVID vaccine numbers is required.

Patient Care and Responsibilities

The Occupational Therapist will manage a case load of 8-12 patients per week, primarily working with adults and geriatric populations. Responsibilities will include:

• Conducting assessments and evaluations.
• Developing and implementing individualized treatment plans.
• Utilizing adaptive equipment and recommending safety measures.
• Providing patient and family education.
• Collaborating with interdisciplinary teams to enhance patient care.

Qualifications

• A minimum of 1 year of experience as an Occupational Therapist.
• Experience with a variety of diagnoses and trauma events is preferred.
• Preferred skills include assessment, cognitive rehabilitation, and home health care experience.
