Compassionate Hospice RN Case Manager

Gentiva Health Services, Inc. is seeking an RN Case Manager to deliver compassionate hospice care in patients' homes across Indiana. You will perform comprehensive assessments, develop and update plans of care, and coordinate with an interdisciplinary team to ensure safe, effective and patient-centered service.
