LPN/LVN - Provide Compassionate Care to Patients at Avir at John Jones

We are seeking a dedicated and compassionate Licensed Practical/Vocational Nurse (LPN/LVN) to join our team at Avir at John Jones. As an LPN/LVN, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients.

About the Role:

As an LPN/LVN, you will be responsible for providing direct patient care under the supervision of a licensed nurse. You will work closely with our multidisciplinary team to develop and implement individualized care plans, monitor patient progress, and maintain accurate and up-to-date medical records. Your exceptional communication skills will enable you to effectively educate patients and their families on treatment options, medication, and other essential health information.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians

• Develop and implement individualized care plans in collaboration with the healthcare team

• Monitor patient progress, identifying and reporting any changes or concerns to the nursing team

• Maintain accurate and up-to-date medical records, including patient intake, treatment, and discharge information

• Communicate effectively with patients, families, and healthcare team members to ensure seamless patient care

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Current LPN/LVN license in the state of Texas

• 0 years of experience in a healthcare setting

• Ability to work effectively in a fast-paced environment
