This role is part of a certified Level II Trauma Center and integrated network, emphasizing a team-based approach to deliver high-quality, comprehensive care. You will collaborate with care coordinators, behavioral health specialists, educators, and medical specialists to manage chronic conditions, treat acute illnesses, and provide preventive services.
