Responsibilities

1. Deliver direct nursing care in accordance with physician orders and individualized care plans.

2. Administer medications and treatments safely and accurately.

3. Monitor, assess, and document patient conditions and report changes promptly.

4. Assist with admissions, transfers, and discharges.

5. Perform wound care and other nursing procedures within the LPN scope of practice.

6. Collaborate with physicians, Registered Nurses, CNAs, therapists, and interdisciplinary teams.

7. Maintain accurate and timely electronic medical documentation.

8. Follow infection prevention, OSHA, HIPAA, and facility policies.

9. Educate patients and families regarding treatments and care plans when appropriate.

10. Respond appropriately during emergencies.

11. Promote patient dignity, comfort, and quality of life.

Qualifications

* Current unrestricted Pennsylvania LPN License.

* Current CPR/BLS Certification.

* Graduate of an accredited Practical Nursing program.

* Excellent communication and interpersonal skills.

* Strong organizational and time management abilities.

* Ability to work independently and within interdisciplinary teams.

* Reliable transportation preferred.

Preferred Qualifications

* Minimum of one (1) year of clinical nursing experience preferred.

* Experience in long-term care, rehabilitation, home health, behavioral health, or skilled nursing facilities is a plus.

* Experience with electronic medical records (EMR) systems.

Physical Requirements

* Ability to stand, walk, bend, and lift up to 50 pounds.

* Ability to assist patients with mobility and activities of daily living.

* Ability to work in fast-paced healthcare environments.
