Field RN (Hospice), Per Diem, Day Shift

Job Summary Plan, organize and direct home care services to meet the needs of patients in their homes. Manage a patient caseload efficiently. Provide patient care independently while collaborating with core disciplines per the patient’s plan to meet care goals. Serve as a resource to patients, family members, and other care team members. Job Requirements Education and work experience: Bachelor’s Degree in Nursing (BSN) – Preferred Experience in an acute care facility or home care – Preferred Registered Nurse (RN) licensure in the state of practice – Required Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification – Required Valid driver’s license (DL) – Required (must be at least 21) Current automobile insurance and a reliable vehicle – Required Essential Functions Complete initial and ongoing comprehensive assessments of patient needs at appropriate time points. Conduct nursing assessments for assigned patients. Discuss and teach nursing care and patient rights and responsibilities. Identify and report problems, abnormal findings, or nursing diagnoses requiring follow‑up. Implement plan of care based on identified needs. Participate with patients in clarifying mutually agreed goals and incorporate them into the care plan. Provide direct and/or indirect skilled, safe, quality care. Coordinate patient care with other disciplines and integrate interprofessional input. Perform ongoing nursing assessments and adjust plans as needed. Accurately document all pertinent information in accordance with policies and procedures. Ensure timely completion and submission of documentation. Update nursing plans and medication profiles when changes occur. Counsel and educate patients and families regarding healthcare, nursing, and related needs. Offer learning opportunities for patients and caregivers, including written materials. Initiate appropriate preventive and rehabilitative nursing procedures. Comply with applicable legal requirements, standards, policies (e.g., HIPAA), and documentation standards. Demonstrate professional conduct, service excellence, and commitment to the mission and values of Adventist Health. Perform on‑call duties as assigned based on location and agency operational needs.
