The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Registered Nurse Case Manager in the Specialty District. DDD provides individuals with developmental disabilities and their families with services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision‑making and engagement in the community. Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor. Job Duties Convene and facilitate in‑person, multi‑disciplinary Person‑Centered Service Plan (PCSP) meetings to determine and identify services. Coordinate and monitor member long‑term care, physical and behavioral health services. Responsibilities include vendor calls for long‑term care services, CRS referrals, specialized transportation, discharge, specialty care, arranging for medical equipment, and coordinating skilled nursing or home health services. Complete additional documentation pertaining to care coordination of physical and behavioral health needs of Division members. Attend staffing and discharge planning meetings and meet with health plan and health care professionals to network to provide continuity of care for members. Conduct assessment of member’s physical, behavioral, and long‑term care needs to develop comprehensive care plans for prevention, outcome improvement, decreased risk status, and increased self‑care opportunities. The assessment includes Social Determinants of Health; social and economic factors, natural and built environments, and policies and programs. Write comprehensive progress notes and maintain case records. Conduct on‑site visits to member/family homes, schools, and program sites to discuss member’s progress, needs, and develop relationships. Become familiar with family dynamics, needs, and supports and make appropriate referrals to community agencies. Provide oversight relating to services the member receives and report quality of care concerns promptly per DDD’s policies and procedures. Assist with members/families in crisis. Assess nutritional status and needs of member and make referral to RBHA, dietician, primary care physician, or health plan, as appropriate. Assist in assigned areas to complete audits/fact‑finding reviews; monitor and complete cost‑effective studies on assigned members. Prepare a variety of reports including a Licensed Health Aid Hourly Nursing Assessment Tool. Knowledge, Skills & Abilities (KSAs) Knowledge in: Managed Care, Medicaid and Medicare concepts Case management concepts, principles, and strategies Nursing theories and practices related to individuals with developmental disabilities and behavioral health conditions Interdisciplinary team planning processes Person‑centered planning strategies Skills in: Case management (interviewing, advocacy, collaboration) Comprehensive nursing assessment and needs analysis Building consensus, continuous quality improvement, and financial accountability Critical interviewing techniques Developing, analyzing, and implementing health treatment plans Abilities: Work with other physicians and health professionals Work with the legal system Observe and assess health status and needs of individuals with developmental disabilities and behavioral health conditions Prioritize and multi‑task to manage workload and meet timelines Interpret and apply DDD policies and procedures Selective Preference(s) BSN from an accredited college or university. Two years of experience working as a registered nurse in behavioral health, home care, hospice or long‑term care (highly desirable). Knowledge of the current Medicaid and Behavioral Health service delivery system is a plus. Pre‑Employment Requirements Possession of a current license to practice as a registered nurse in the State of Arizona at time of appointment. Any employee operating a motor vehicle on state business must possess and retain a current, valid class‑appropriate driver’s license, complete all required training, and successfully pass all necessary driver‑license record checks. Employees who drive on state business must maintain acceptable driving records and complete required driver training. Employees who operate a personally owned vehicle on state business must maintain the statutorily required liability insurance. Valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41‑1968 for work with children and vulnerable adults. Search of the Department of Child Safety Central Registry and the Adult Protective Services Registry. Pass background and reference check; employment is contingent upon completion of the above‑mentioned process and the agency’s ability to reasonably accommodate any restrictions.
