City/State: Hampton, VA Work Shift: First (Days) Status: Full-time, permanent position (40 hours) Standard working hours: 8am to 5pm EST, M-F Location: Send to Sentara Careplex in Hampton, VA daily. Overview The behavioral health navigator level III serves as a senior member of the care coordination team, providing expert support for high‑acuity behavioral health cases. This role includes workflow leadership, mentoring, and project participation to enhance service delivery across the continuum. Responsibilities The behavioral health navigator level III serves as a senior member of the care coordination team, providing expert support for high‑acuity behavioral health cases and leading workflow, mentoring, and project initiatives to improve care delivery. Education Master’s Degree in Social Work, Counseling, Clinical Mental Health Counseling, Marriage and Family Therapy, Psychology, Addiction Studies, Nursing, or a closely related field OR Bachelor’s of Science in Nursing for RN Candidates Certification/Licensure Basic Life Support (BLS) – American Heart Association (AHA) certification required within 90 days of hire. Experience Eight years of healthcare experience delivered in one of the following settings: Acute care, long‑term care, home health, hospice, skilled nursing facility, or community‑based program services.
