Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and develop tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work in conjunction with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
• Provide education to patients and families concerning therapeutic goals, discharge plans, and home exercise programs
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure a seamless provision of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are a plus, but not necessary

Skills

• Previous experience is highly desirable
