Responsibilities:

• Lead and manage a team of social workers , overseeing daily operations and ensuring that residents receive timely and effective support

• Develop, implement, and maintain social services programs , including assessment, care planning, and discharge planning

• Collaborate with interdisciplinary teams , including nursing, therapy, and administration to ensure comprehensive care and services

• Maintain accurate records and reports , including social work notes, care plans, and other required documentation

• Participate in organizational quality improvement initiatives , contributing to the development and implementation of quality care initiatives

• Stay up-to-date with industry trends and regulations , ensuring compliance with relevant laws and standards

Requirements:

• Minimum 2+ years of experience in social work, preferably in a healthcare setting

• Knowledge of social work principles, theories, and practices

• Effective communication, leadership, and problem-solving skills

• Ability to work in a fast-paced environment with multiple priorities and deadlines
