Clinical Pharmacist

Job Summary and Qualifications Provision of care: Coordinates and assists with development, implementation and maintenance of clinical pharmacy programs to ensure clinical services meet patient population and community requirements. Provides clinical pharmacy support and executes clinical pharmacy programs for all patient areas. Reviews medication orders and quality of technicians work to ensure appropriateness of drugs, dosage, route and interactions. Adjusts medication dosage/schedule as appropriate for patient condition per policy. Ensures medications are safely dispensed in accordance with age‑specific guidelines. Follows procedures for sterile preparation of IV admixtures in compliance with USP 797 standards. Participates in evaluation and analysis of clinical data, medication use, drug and drug class monograph development in support of clinical toolkit development. Provides oversight of pharmacy distribution activities including order receipt, product storage, controlled substances process, dispensing and other functions. Closely monitors narcotic transactions and reconciles quantity/type of narcotics. Assists department in addressing physician consults and provides timely retrieval of reliable drug information. Patient & Family Communication: Communicates and interacts with patients, families and others in a caring, empathetic, courteous, and respectful manner. Physician Communication & Coordination: Communicates effectively and works with physicians and/or other interdisciplinary team members to meet patient needs. Receives, interprets and implements physician orders accurately and courteously. Consults resources to clarify unfamiliar or questionable orders. No travel required.
