Shift: Monday - Friday | 8:00 AM - 4:30 PM As a Physical Therapist (PT) on our Home Health team, you'll bring expert care and support to patients where they feel mostcomfortable-their own homes. Responsibilities • Makes visits to the homes of patients requiring home health physical therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient. • Performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures. • Evaluates and assesses the patient's level of functioning by applying appropriate tests. • Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. • Initiates diagnostic, preventative and rehabilitative procedures as appropriate to the patient's care delivery Qualifications Education: Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association. Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided. Licensure/Certifications: Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation. Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements. Mandatory Reporter: Child & Dependent Adult Abuse. CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification. Minimum Experience: One year experience caring for similar patient population as to be assigned.
