Licensed Practical Nurse (LPN) — Skilled Nursing / Long-Term Care

Location: Honesdale, PA 18431 (Wayne County) Employment Type: Full-Time | Part-Time | Per Diem Pay: Up to $34.40/hr

Position Summary

A skilled nursing and rehabilitation facility in Honesdale, PA is seeking Licensed Practical Nurses to join its nursing team. The LPN delivers direct resident care under the direction of the RN Supervisor / Director of Nursing, administers medications and treatments, documents accurately, and helps supervise nursing assistants on the unit. This is a hands-on floor role in a resident-centered, survey-ready building.

Key Responsibilities

• Administer medications and treatments per physician orders and facility policy; complete accurate MAR/TAR documentation
• Perform routine nursing procedures: wound care and dressing changes, catheter care, ostomy care, tube feedings, glucose monitoring, oxygen therapy, nebulizer treatments
• Complete admission, readmission, and change-of-condition assessments; report findings promptly to the RN Supervisor or provider
• Monitor and document vital signs, intake/output, skin condition, behavior, and response to interventions in the EHR (PointClickCare or comparable)
• Supervise and direct CNAs on assigned unit, including assignments, break coverage, and follow-through on care plans
• Communicate with residents, families, physicians, pharmacy, and interdisciplinary team members professionally and promptly
• Respond to falls, emergencies, and change-of-condition events; initiate protocols and complete incident documentation
• Participate in care plan meetings, shift report, and infection control, and support survey readiness at all times
• Maintain compliance with PA Department of Health regulations, CMS requirements, HIPAA, and facility policy

Requirements

• Active, unrestricted Pennsylvania LPN license (or multistate/compact privilege valid in PA)
• Current BLS/CPR certification (AHA preferred)
• Graduate of an accredited practical nursing program
• Minimum 1 year of LPN experience preferred; skilled nursing/long-term care experience strongly preferred
• IV certification a plus (required for some assignments)
• Working knowledge of electronic health records; PointClickCare experience preferred
• Physical requirements: able to lift/assist up to 50 lbs, stand and walk for extended periods
• Must pass background check, drug screen, and PA Act 34/Act 33 clearances; TB screening and immunization records required
