RN Peritoneal Dialysis-Dialysis West-Laveen

Position Summary: Provides dialysis nursing care to end stage renal failure patients in their homes and at the direction of physicians by performing the following duties personally. Responsible for ensuring that all critical tasks are fulfilled on a timely basis. Critical Tasks:
- Maintains exclusive focus upon patients, the quality of their dialysis care, their individual needs and their medical outcomes without significant distraction by administrative and non-patient contact activities.
- Complies fully with all patient medical records and information privacy regulations, state and federal.
- Takes all steps necessary to fully ensure patient/resident and medical data confidentiality and security.
- Assists in the identification, evaluation and selection of home dialysis training candidates and guardians as appropriate for the Home Dialysis Program.
- Educates patients and guardians regarding end stage renal disease and treatments.
- Ensures that patients and residents are comfortable and well cared for according to state and federal regulations as well as contemporary nursing practices.
- Ensures exceptional and very highly responsive nursing care for all patients through personal actions.
- Maintains awareness of comfort and safety needs of patients and provides guidance to patients and guardians on these subjects.
- Performs assessments of the home dialysis candidate's home environment for suitability in the delivery of home dialysis therapy.
- Administers medications as prescribed.
- Provides ongoing education to patients regarding their renal dialysis, peritoneal or vascular access and home dialysis therapy, and other related health conditions.
- Trains home dialysis patients on the safe and effective operation and maintenance of all home dialysis equipment and treatment supplies through an organized and formalized home dialysis training program
- Observes patients and residents, records significant conditions, data and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents. This data may include physician orders, lab results, vital signs, and treatment parameters.
- Coordinates the ordering and delivery of patient's initial dialysis supplies/equipment at initiation of the training program as well as on the on-going operation and preventative maintenance of home dialysis equipment.
- Takes all steps and actions necessary to ensure full enterprise compliance with all state and federal regulations and medical standards.
- Charts patient and resident data in compliance with Medicare/Medicaid requirements while accurately noting abnormal findings and changes in patient condition.
- Travels by automobile throughout the community to patient residences.
- Designs and participates in health awareness and wellness activities with patients.
- Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.
- Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.
- Performs other special projects and duties as assigned.
- Graduate of a Registered Nurse program from an accredited university or college; Bachelor's Degree preferred.
- Registered Nurse Licensure in AZ or qualified Compact Licensure.
- 1 year of experience as a Peritoneal Dialysis Registered Nurse preferred.
- Current and continuing BLS training and certifications.
- State of Arizona driver's license with no significant violations during the last 5 years.
- Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
