Job Responsibilities

• Complete initial client assessments and admission visits (SOC).
• Responsible for performing initial and ongoing competency assessments for Home Health Aides (HHAs) to ensure safe, compliant patient care.
• Report client health status clearly and promptly to the Nurse Supervisor for proper care coordination
• Travel throughout Broward County as needed
• Demonstrate empathy, professionalism, and respect when working with clients and families
• Ensure all documentation is complete, precise, and submitted on time.
• Anticipate potential issues and appropriately escalate or resolve concerns
• Prioritize tasks and demonstrate a strong sense of urgency

Required Experience & Education

• Active Florida Registered Nurse (RN) license
• Current Level 2 clearance in the AHCA Clearinghouse
• TB test within the last 12 months or Chest X-ray within the last 5 years
• Prior home care experience required - Minimum 1-2 years
• Driver's License (Required)
• CPR/BLS Certification (Required)
• Spanish Speaking a plus!
