Title
Home Health LMSW

Schedule
- Shift: 8-hour days
- Weekends: Not required

Assignment Details
- Patient ratio: 1:1 (may visit up to 6 patients per day)
- Guaranteed hours: Facility may call off up to one shift per pay period

Requirements
- Required Licensure: Must have BOTH Oregon and Washington licensure (Washington license may be in application process)
- Required Certification: BCLS/BLS
- Experience: 1 year skilled home health experience
- EMR: OASIS experience required

Roles & Responsibilities
- Conduct biopsychosocial assessments and develop care plans.
- Facilitate care coordination and case management, including discharge planning.
- Complete home health documentation and OASIS entries accurately.
- Navigate Medicare/Medicaid processes and connect patients with community resources.

Compliance / Notes
- Parking: Free
- Dress Code: Business casual; scrubs allowed but not required; no jeans; no specific color required
- COVID-19 Vaccination: Primary series plus one booster required
