Location: Eustis, Florida
Position: Full Time
Salary: $45–$54 per hour

Lake Eustis Healthcare & Rehabilitation is seeking a compassionate and experienced Occupational Therapist to join our rehabilitation team full time. In this role, you’ll provide resident-centered therapy, support recovery goals, and contribute to a positive, collaborative care environment.

Key Responsibilities

• Collaborate with nursing, administration, and other departments to provide effective therapy services.

• Develop, implement, and maintain individualized occupational therapy care plans.

• Conduct resident evaluations, progress reviews, care planning, and discharge coordination.

• Maintain accurate documentation of therapy sessions, resident progress, and treatment outcomes.

• Ensure therapy practices align with current clinical standards and regulatory requirements.

• Provide guidance and support to therapy staff in collaboration with the Director of Rehabilitation.

• Participate in quality assurance and performance improvement initiatives.

• Communicate effectively with residents, families, and interdisciplinary team members.

• Complete administrative duties, including documentation, reports, and billing accurately.

Qualifications

• Active and valid Florida Occupational Therapist (OT) license.

• Minimum 1 year of experience in skilled nursing or long-term care preferred.

• Strong knowledge of therapy regulations, documentation standards, and rehabilitation best practices.

• Excellent communication, organization, and clinical reasoning skills.
