This unique role offers the opportunity to serve as the sole transplant pharmacist specialist, providing direct impact and leadership within a collaborative, interdisciplinary team.\n \nOur transplant program services kidney and liver transplantation with an average of 9-10 transplants per month.

In this position, you will work closely alongside an experienced Nurse Practitioner, transplant surgeons, and medical residents, contributing to patient care through medication management, immunosuppression optimization, education, and clinical decision‑making. Standard working hours are Monday through Friday, 8 hours per day, with a variable start time based on the rounding team.\n The latest permissible start time is 8:00 am.\n The role includes a weekend rotation every fourth weekend, with hours from 7:00 am to 3:30 pm.\n The position requires working two holidays per year.\n There is no on-call requirement.\n\n\nSkills & Matching Criteria: \n\n Solid organ transplant experience is needed.\n Acceptable qualifications include either Post Graduate Year 2 (PGY2) training in transplant or prior work experience in solid organ transplant.\n\n\n\nApply today. SSM Health- We bring out the incredible in you, every day.\n\n\n\nJob Summary:Optimizes patient care by promoting rational, safe, and cost-effective drug therapy by developing and providing clinical services in their area of expertise. Conducts drug therapy monitoring, drug formulary reviews, and makes pharmaceutical interventions with the goal of reducing adverse events. Educates and trains pharmacy staff, pharmacy residents, students and other members of the healthcare team as appropriate for the site.\n\n\n\nJob Responsibilities and Requirements:\nPRIMARY RESPONSIBILITIES\n\n Provides pharmaceutical care in their specialty practice area which may include: review and evaluation of patient drug/medical history, providing drug therapy recommendations to optimize care, providing drug information, evaluating medical literature, rounding with other healthcare providers, drug utilization reviews, patient counseling and order entry/verification/dispensing.\n Plans, implements, performs and measures the clinical processes which improve treatment outcomes, minimize adverse drug events and contributes to quality and financial outcomes. Coordinates DUEs, analyzes trends and evaluates data to be synthesized into reports for action by committees.\n Creates a culture of learning and sharing that supports the development of a qualified clinical staff through coaching, motivation, and empowerment. Implements staff development on clinical processes and participates in the training of pharmacists, pharmacy residents, pharmacy students, and healthcare providers as appropriate for the site.\n Promotes interdisciplinary teamwork including development and implementation of clinical pathways and protocols that promote appropriate medication safety and medication utilization using best practice, evidence-based recommendations.\n Works in a constant state of alertness and safe manner.\n Performs other duties as assigned.\n\n\n EDUCATION\n\n Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy\n\n\n EXPERIENCE\n\n Two years experience or completion of PGY1 residency\n\n\nPHYSICAL REQUIREMENTS\n\n Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.\n Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.\n Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.\n Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.\n Frequent keyboard use/data entry.\n Occasional bending, stooping, kneeling, squatting, twisting and gripping.\n Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.\n Rare climbing.\n\n\nREQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS \n\nResuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.\n
