Occupational Therapist – Home Health

Contract | Days | Full-Time (5x8-hour shifts, 8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled occupational therapy services to home health patients across Hood River, OR and surrounding areas
• Conduct evaluations and deliver therapeutic interventions for patients with diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke conditions
• Complete OASIS documentation accurately and in a timely manner using EPIC Remote Client and Rover mobile application
• Support patients with activities of daily living, functional activities, and both acute and chronic disorder management
• Visit up to 6 patients per day, with occasional travel up to approximately 60 miles per day within the service area
• Maintain productivity standards in accordance with home health program expectations

Required Qualifications

• Active licensure in both Oregon and Washington State (WA license in hand preferred; OR license may be pending at time of submission)
• Minimum 1 year of skilled home health occupational therapy experience
• OASIS documentation experience required
• Current Basic Cardiac Life Support (BCLS) certification
• Reliable personal transportation required for daily patient visits

Preferred Experience / Skills

• Familiarity with National Patient Safety Goals and Core Measures
• Experience with home health billing processes
• Proficiency with OASIS documentation workflows and home health-specific charting platforms
• Experience supporting patients with functional activities and chronic or acute disorder management
