The Case Manager manages and coordinates interdisciplinary care for defined populations through the continuum from wellness to end of life. The role focuses on clinical, psychological, social, and environmental perspectives to assess and support patient needs. High-risk patients will require ongoing coordination of care. A comprehensive patient and family assessment will be performed to develop treatment plans addressing clinical and psychosocial issues. The Case Manager implements care plans that provide continuing support for patients with complex needs. The position involves managing benefits and negotiating continuing care services within various health insurance plans. Innovative strategies will be utilized to advocate for patient needs and help remove barriers to accessing healthcare. Monitoring patient transitions across different care settings, such as home, clinic, and skilled nursing facilities, is essential. The Case Manager will share assessment and care plan data with patient and family consent during transitions. Identifying and addressing gaps in the care continuum is crucial, as is collaborating with community and provider networks to expand access to necessary services. Participation in developing clinical disease management strategies and evaluating outcomes is expected. The role includes collecting and evaluating clinical and financial data, including measures of patient satisfaction and resource utilization. Requested time off during the assignment must be included upfront, as late requests will not be considered. Local candidates within 50 miles of the facility are not eligible for this position. Candidates must have their own vehicle for transportation as public transport is not available in the area. All scheduling requests must be made at the time of submission.
