RN Case Manager Job Details

Location: Sacramento, CA 95819 (Northern California nursing jobs)
Healthcare Setting: On-Site
Employment Type: Travel RN Contract
Contract Duration: 13 weeks
Estimated Weekly Pay: $2,629 - $2,921
Hours per Week: 40
Shift: 8 Hour Days
Schedule: Monday to Friday, 0800-1630, with every other weekend rotation
Electronic Medical Record: Cerner EMR

Registered Nurse Qualifications

Active California RN license
Minimum 1 year of clinical nursing experience
Case Management nursing experience preferred
Experience with Cerner EMR preferred
Strong communication and organizational skills
Ability to work every other weekend as part of the rotation

RN Case Manager Responsibilities

Coordinate patient care and discharge planning with multidisciplinary healthcare teams
Advocate for patient needs and facilitate access to healthcare resources
Utilize Cerner EMR for accurate documentation and care coordination
Participate in daily case management rounds and collaborate with physicians, social workers, and other healthcare professionals
Ensure compliance with regulatory requirements and hospital policies
Provide compassionate support to patients and families throughout their healthcare journey
