Competitive pay 401(k) with generous company match Employee referral program Merit-based pay increases Employee discount programs Supportive team culture and healthy work-life balance Key Responsibilities Evaluate patients and develop individualized treatment plans based on physician orders Provide skilled physical therapy services in the patients home Help patients improve mobility, strength, balance, and functional independence Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques Recommend adaptive equipment and home modifications to promote safety and independence Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes Complete accurate and timely clinical documentation and care plan updates Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations Encourage and motivate patients throughout their rehabilitation journey Qualifications Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) Current Physical Therapist (PT) license in the state of Tennessee Home Health experience preferred, but not required Current CPR certification Valid drivers license, reliable transportation, and proof of automobile insurance Apply Today!
