Duties Include Assess residents’ psychosocial, emotional, and discharge planning needs Develop and implement individualized social service interventions Coordinate discharge planning and transitional care services Assist residents and families with care planning and community resources Facilitate advance directive discussions and decision-making Coordinate Family Council meetings and family support initiatives Manage resident grievance and complaint processes Ensure compliance with state, federal, and company regulations Collaborate with interdisciplinary healthcare teams to enhance resident quality of life Advocate for resident rights, dignity, and emotional well-being Required Qualifications Qualifications for Social Services Manager Bachelor’s Degree in: Social Work (BSW) Human Services Psychology Sociology Gerontology Rehabilitation Counseling Special Education or related field Minimum one (1) year of supervised social services experience in a healthcare setting Experience working with elderly residents or geriatric populations Strong communication, documentation, and organizational skills Preferred Qualifications Licensed Social Worker preferred (LBSW, LSW, LMSW, LCSW, etc.) Prior Skilled Nursing Facility (SNF), Rehabilitation, Long-Term Care, Nursing Home, or Healthcare Social Services experience strongly preferred Our Service Standards Sincere Appreciation...for everyone Hearing Needs...and responding quickly Caring Approach...in everything we do Managing Situations...for positive outcomes
