Certified Home Health Aide

Certified Home Health Aide (CHHA) position with a focus on providing personal care and supportive services to patients in a home setting. Benefits 401K Matching Plan – Up to 4% Must be available a minimum of 3+ days a week Description of Responsibilities The Certified Home Health Aide (CHHA) is responsible for delivering personal care and assistance to patients within the home environment under the supervision of a Registered Nurse or Therapist. The role includes assisting with activities of daily living, monitoring patient condition, maintaining a safe support environment, and providing administrative support during periods of low census. Reporting Relationship Clinical Supervisor Scope of Supervision None Responsibilities Providing hands‑on personal care including but not limited to: Baths Oral hygiene Assisting patients with dressing and undressing Skin care to prevent breakdown Assisting the patient with toileting activities Keeping patient’s living area clean and orderly, as appropriate Planning and preparing nutritious meals. Assisting in feeding the patient, if necessary. Taking and recording oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered. Performing range‑of‑motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency). Reporting on patient’s condition and significant changes to the assigned nurse. Minimum Qualifications Current experience providing patient care in a home health, healthcare, or caregiving setting preferred. One (1) year of Home Health experience preferred. Previous administrative or clerical experience in a healthcare setting preferred. One (1) year of experience with EMR systems preferred; CareTend, KanTime, or CPR+ experience preferred. Strong verbal and written communication, organizational, and documentation skills required. Education and/or Experience Current Certified Home Health Aide (CHHA) certification in the State of California required. Valid California driver’s license, reliable transportation, and current automobile insurance required.
