RN Case Manager

RN Case Manager Position Summary The RN Case Manager is responsible for coordinating and overseeing the delivery of rehabilitation and medical services for residents in a Skilled Nursing Facility. This role collaborates closely with the interdisciplinary team to ensure cost-effective, high-quality care while facilitating admissions, ongoing utilization review, and discharge planning. The RN Case Manager serves as a resident advocate and primary liaison between the facility, insurance payers, and external medical management professionals.
