Travel Nurse RN - ICU - Intensive Care Unit

No candidates with an AZ home address on their drivers license will be considered - please include a copy in submission Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidate are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quick! This form needs to be 100% accurate and have all request noted in the highlights section. Start Date should be on or very quickly after the Order Start Date listed. All RTO should be listed on the cover page, must be less than 7 days. RTO will not be approved in the first 2 weeks of assignment. All facilities the traveler is open to working in should be listed on the submission. If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.) All shifts the traveler is open to working on should be listed on the submission. All departments the traveler is open to working must be checked on the cover page. All candidates submitted should be preclosed and ready to accept. Please let us know if the traveler withdraws or is no longer available.

SUBMISSION REQUIREMENTS

• No candidates with an AZ home address on their drivers license will be considered - please include a copy in submission
• Minimum 24 months of current Level I Trauma ICU experience - REQUIRED
• MICU experience required
• Must have previous continuous ICU experience for the last 4 years - REQUIRED
• May float to the other ICU units as needed, NSICU, CVICU and STICU - REQUIRED
• Certifications must be American Heart Association OR client and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED
• No Nursys Hits - REQUIRED
• Banner will not approve RTO within the first two weeks or holidays. Can not exceed 7 days.
• Flu vaccination - Banner declinations can be applied for (reach out for forms)
• ***PLEASE NOTE, BANNER HAS VERY SPECIFIC BACKGROUND REQUIREMENTS THAT MUST BE ADHERED AND ATTESTED TO. PLEASE LOOK AT THE COMPLIANCE REQUIREMENTS IMMEDIATELY***

• Resume with all facilities, facility size, beds, trauma level, and units listed
• Skills Checklist specific to the discipline (must be less than a year old)
• Active License (verification must be less than 30 days old)
• Certifications (must be active at time of submission)
• Flu vaccination - Banner declinations can be applied for (reach out for forms)
• Driver's License

CERTIFICATION REQUIREMENTS

• ACLS (AHA)
• BLS (AHA)

STATE LICENSE REQUIREMENTS

• Arizona Compact Allowed
