LTSS Service Coordinator - RN Clinician

LTSS Service Coordinator – RN Clinician Location: San Jacinto, Montgomery, or Trinity counties in Texas. Candidates must reside in one of these counties and be willing to travel up to one hour daily. This field‑based role requires frequent travel to client sites and occasional office attendance. Responsibilities Overall management of member’s case within scope of licensure, including supervision and direction to non‑RN clinicians as appropriate. Develop, monitor, evaluate, and revise care plans to meet member needs and optimize health care across the continuum. Perform face‑to‑face clinical assessments to identify, evaluate, coordinate, and manage members’ physical, behavioral, social, and long‑term service needs. Identify high‑risk members, coordinate care with the health‑care team, and ensure timely interventions. Manage members with chronic illnesses, co‑morbidities, and/or disabilities, ensuring cost‑effective use of benefits. Obtain thorough member history to develop individualized care plans and set short‑ and long‑term goals in collaboration with members, caregivers, families, and providers. Develop and oversee service access, including authorizations and referrals within benefit structures or extra‑contractual arrangements when permissible. Interface with Medical Directors, Physician Advisors, and inter‑disciplinary teams to develop treatment plans and solve issues with providers, claims, or services. Direct and supervise work of LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals, assigning tasks, reviewing assessments, and providing performance input. Minimum Requirements Registered Nurse with current, unrestricted RN license in applicable state(s). Minimum 3 years experience in a Service Coordinator, Case Management, or similar role working with individuals with chronic illnesses, co‑morbidities, and/or disabilities. State‑specified certification may be required by law or contract. Preferred Skills, Knowledge, and Experience Master’s or advanced degree in Health/Nursing. Willingness to travel to worksites and other locations as necessary.
