Job Title Outpatient Therapist – Bachelor’s/Master’s Level (Mental Health Worker) Program(s) Outpatient Clinic/School Based Mental Health Location Pennsylvania Reports To Clinical Supervisor FLSA Status Non‑Exempt Summary This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload and for ensuring completion of required clinical documentation in a timely manner. Essential Duties and Responsibilities Conduct psychosocial assessments at intake and at ongoing intervals, completing standardized assessments and/or outcome‑based measures. Develop individualized treatment plans in accordance with applicable county, state, and federal regulations. Provide therapy in the settings recommended by individual consumer treatment plans. Collaborate with all team members to deliver comprehensive, whole‑person treatment. Continuously evaluate treatment efficacy through review of individualized consumer‑driven plans. Create transition and discharge plans based on recovery progress, goals, and symptomology. Complete all required clinical documentation, including progress notes, discharge summaries, incident reports, and treatment reviews through the electronic health record system. Respond to emergency or crisis situations with team members and consumers, ensuring safety and applying de‑escalation skills as needed. Maintain an assigned, planned schedule by following program guidelines to ensure regular service provision. Coordinate care through routine conversations and referrals to natural supports, external agencies, PCPs, school districts, and others. Engage in ongoing supervision, training, and department and team meetings to support professional development. Adhere to professional and ethical interaction policies, behaving respectfully with consumers, family members, and coworkers. Assist with additional duties to support business priorities and processes. Other Duties While this description outlines primary responsibilities, additional duties may be assigned as needed. Requirements Required Education and Experience Bachelor’s degree from an accredited college or university in Psychology, Social Work, Nursing, Rehabilitation, or Activity Program. OR a graduate degree in a clinical discipline with at least 12 graduate‑level credits in mental health or counseling. Preferred Education and Experience Minimum of one year of experience with a CASSP service system, or employment by a licensed mental health services agency, or other mental health clinicians with training in behavioral modification techniques. Certificates, Licenses, Registrations None required. Qualifications Proficient in electronic health record systems. Strong ability to read, analyze, and interpret complex documents and instructions. Effective communication skills, both written and oral. Mathematical proficiency, including basic arithmetic and use of MS Excel for data entry and reporting. Sound reasoning ability to make calm, timely decisions and to utilize a variety of therapeutic modalities tailored to consumer needs. Team‑working skills and organizational competence. Experience with psychological assessments and outcome measures. Travel Up to 100% travel between appointment sites or other locations, as outlined by the program. Physical Demands May be required to remain stationary 50% of the time. Occasional movement in home, school, community, clinic, or program office environments; including use of computers, copying, printing, and occasionally ascending or descending stairs. Must communicate frequently with consumers, employees, and external sources and manage resources and materials in diverse settings. Work Environment Work may occur in home, school, community, clinic, or program office settings as specified by individual consumer treatment plans.
