LVN Clinical Coordinator

Summary of Essential Functions for the Position:The Clinical Coordinator LVN is responsible for reviewing, analyzing, uploading, and completing lab results and orders that are in the processing center folders. Provide support to the clinical field staff, patients, caregivers, and medical providers in a timely and effective manner.Qualifications / Licensure / Certification / Knowledge / Skills / Abilities:Currently licensed in the state(s) in which practicingCPR certified with American Heart Association or American Red CrossNursing experience with emphasis in home care preferredDemonstrates knowledge of Medicare, accreditation criteria and standardsAcceptance and ability to demonstrate support the core values and goals of AgencyAbility to demonstrate flexibility, initiative, and work with minimal supervisionAbility to work with individuals to enlist cooperation of many people to perform/achieve a common goalAbility to demonstrate critical thinking, observation, good written and oral communication skillsAbility to demonstrate problem identification & resolution skillsAbility to demonstrate positive customer service skillsMust have a current drivers license, auto liability insurance and reliable transportationMust be on call at least once per month, or as neededTravel may be required.Educational Requirements:High School Diploma or GED. Graduate from an accredited school of nursing.Working Conditions & Physical Requirements:Work environment is relatively busy and can be stressful at times.
