Care Manager: Behavioral Health & Whole-Person Care

ncgCARE is hiring a Care Manager in Bryson City, NC to proactively engage high-risk members and coordinate their physical, behavioral, and support needs. You will assess, evaluate, and develop ISP plans, manage medications, and provide self-management tools beyond visits with providers.

The role focuses on assertive outreach, building rapport with hard-to-reach individuals, and meeting productivity standards in a performance-driven environment.
