RN Case Manager (RN) - Home Health

Registered Nurse Case Manager (RNCM) Assumes overall accountability and leadership for assessing patient condition and developing a comprehensive plan of care. Provides case management and clinical care while coordinating all disciplines to ensure quality, cost‑effective, and billable care. Schedule: Monday‑Friday 8:00AM‑5:00PM; on‑call for two weekdays and two weekends each month. Location: Indianapolis and surrounding areas. Sign‑on Bonus: $10,000. Key Responsibilities Oversee patient care from start to finish of each episode. Update physician orders in the patient chart as needed. Notify physicians of any significant changes in patient condition. Manage and coordinate multidisciplinary care, as applicable. Provide skilled nursing care in the patient’s home as ordered by the physician. Observe and monitor patient conditions. Perform OASIS assessments to develop individualized care plans and adjust as needed. Administer medications as prescribed. Reduce re‑hospitalizations through proactive interventions, including front‑loading visits for high‑risk patients. Promote continuity of care through appropriate admissions, transfers, and discharges. Educate patients and families on disease processes and self‑management strategies. Involve patients and families in care plan development. Teach preventive measures to reduce re‑hospitalizations. Listen and respond to patient and family concerns. Supervise care provided by LPNs and nurse aides. Evaluate patient condition and treatment effectiveness against the plan of care. Determine whether current care levels meet patient needs. Communicate supervisory visit findings with LPNs and nurse aides. Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines. Document all treatments, assessments, and patient education. Maintain an active RN license. Communicate schedule changes with the scheduler. Participate in on‑call rotations and case conferences. Attend required in‑services and agency meetings. Travel to patient locations as scheduled. Maintain consistent daily attendance at assigned visit locations. Complete documentation for subsequent visits, discharges, evaluations, recertifications, and resumptions of care within 24hours. Complete start‑of‑care documentation within 24hours. Submit all required documentation within company‑defined timelines. Follow the plan of care within RN scope of practice regulations. Perform RN visits within assigned geographic areas per patient needs and regulatory requirements. Perform other duties as assigned. Qualifications Active Registered Nurse (RN) license in the state of employment. Minimum of one (1) year of RN experience in an acute care setting; home health experience preferred. Valid driver’s license and auto insurance with the candidate listed as a driver. Strong communication skills with the ability to build positive relationships with patients, families, physicians, and colleagues. Ability to remain calm, patient, and compassionate in a clinical setting. Knowledge of nursing best practices. Sound clinical judgment and decision‑making skills. Ability to assess situations and implement appropriate solutions. Strong attention to detail and observational skills. Strong organizational skills with the ability to prioritize and meet deadlines.
