Nurse Practitioner - Palliative Care

Overview Klarus Hospice & Palliative Care is seeking a PRN (per diem) dedicated and professional Nurse Practitioner to join our team serving patients enrolled in our palliative care services in the San Antonio, TX area, performing face-to-face visits. Responsibilities Assist in overseeing the clinical care and treatment of patients and families; consult with the patient’s attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group (IDG) to ensure quality care. Collaborate with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease. Consult with and assist in the IDG’s development of patient/family plan of care to assure quality care is provided. Educate patients and families regarding disease process, self-care techniques, and end-of-life care; provide training to other staff as needed. Perform and document visits to patients/families based on plan of care and patient/family needs. Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician. Consult with and provide education to other IDG members on a PRN basis for clinical issues related to palliation. Actively participate in the IDG; communicate effectively, timely, and appropriately at all times. Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care. Qualifications Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. Licensure: Current State license as a Registered Nurse and as an Advanced Practice Nurse. Current Driver’s License. Experience: Two years of experience as an APN; clinical care setting preferred; hospice or home health preferred. Skills: Nursing skills as defined by generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life. Good interpersonal skills. Transportation: Reliable transportation and valid auto liability insurance. On-call: 1-2 weeks per month. Environment Works in patients’ homes in varying conditions; possible exposure to blood and bodily fluids and infectious diseases. Requires the ability to work a flexible schedule, travel locally, and occasional exposure to unpleasant weather; PRN emergency call may be required.
