Family Practice - Geriatrics Physician
GHR Healthcare
Contract: 4 6 Months (Minimum)
Schedule: Monday Friday, 8:00 AM 5:00 PM
Location: On-site Day Center
Patient Population & Responsibilities:
Panel Size: participants per provider
Daily Census: participants on-site
Visits: 3 4 patient visits per day (up to 45 minutes each), scheduled for participants
Participant Age: Primarily adults 55+
Common Conditions: Dementia, substance use, and complex mental health disorders
Peak Days: Tuesday Thursday (heaviest patient volume); lighter on Monday and Friday
Additional Details:
EMR: Transitioning to Athena, go-live August 1, 2025 (new providers trained on Athena)
On-Call: Shared among all providers; mostly phone triage with vendor support. Occasional in-person visits to SNFs or ERs may be required
Credentialing: Streamlined process; competency sign-off by Regional Medical Officer required prior to independent practice
