Program Director / Designated Manager (245D) Seasons Adult Foster Care Position Overview The Program Director serves as the Designated Manager (DM) under MN Statute 245D, providing leadership, regulatory oversight, and compliance across all residential programs. This role ensures high-quality, person-centered services for adults with serious and persistent mental illness (SPMI) and co-occurring conditions while maintaining full compliance with licensing standards. Our population includes many with psychotic and personality disorders with a focus on a holistic approach that includes medication management, strong behavioral supports, and nutrition therapy. Reporting to the COO, the Program Director leads the Program Team, including Designated Coordinators, House Leads, and Direct Support Professionals (DSPs), and drives a culture of accountability, quality, and continuous improvement. Key Responsibilities Program Leadership & Operations Oversee daily operations across all residential sites, ensuring services align with CSSPs and SPAs Maintain real-time awareness of program status, staffing, and resident needs Lead admissions, discharges, and program transitions Develop reports, track program metrics, and contribute to strategic planning Partner with HR and leadership on staffing needs and scheduling solutions Designated Manager (245D) Compliance Ensure full compliance with MN Statute 245D, including service agreements, SPAs, and required timelines Oversee assessments, service planning, documentation, and annual reviews Maintain audit readiness and respond to licensing reviews, investigations, and corrective actions Ensure proper incident reporting and adherence to the Vulnerable Adult Act Train and support staff on regulatory requirements and service delivery standards Care Coordination & Case Oversight Coordinate services across providers, agencies, and treatment teams Lead support team meetings and ensure timely follow-up on medical and psychiatric care Monitor individual outcomes and adjust services based on progress and needs Advocate for person-centered supports and appropriate funding/services Quality Assurance & Performance Improvement Conduct audits, monitor documentation, and ensure compliance across programs Identify trends, risks, and improvement opportunities Lead quality improvement initiatives and corrective action plans Ensure consistent, high-quality service delivery and documentation practices Supervise and develop program staff, including coaching, performance management, and accountability Partner with HR on training, discipline, and staff development Provide ongoing training in mental health, de-escalation, and person-centered care Model professionalism, consistency, and strong leadership across all programs Serve as liaison with families, case managers, healthcare providers, and regulatory agencies Build and maintain strong external partnerships Communicate clearly across all stakeholders and advocate for persons served Qualifications Bachelor’s degree in Human Services, Healthcare Administration, or Social Work - or - RN 2+ years of supervisory experience in behavioral health or human services Experience supporting individuals with SPMI and co-occurring conditions Strong knowledge of MN 245D regulations and adult foster care licensing Demonstrated leadership, organizational, and problem‑solving skills Additional Requirements DHS background study clearance Valid driver’s license Ability to participate in on-call support as needed Preferred Experience as a Designated Manager (245D) Familiarity with residential documentation systems (e.g., Residex) Full‑time, exempt position Work across multiple residential sites in Staples, Pillager, and Motley On‑call, evening, or weekend availability as needed Starting Pay $73,000 - $85,000 per year plus benefits - dependent on licensure, education and 245D experience We live our mission through ASPIRE : Accountability, Stewardship, Passion, Integrity, Respect, Empower #J-18808-Ljbffr
