Your Role as a Physical Therapist: As a Physical Therapist at Claremont, you’ll have the opportunity to change lives every day by helping residents achieve their highest level of independence and mobility. You’ll work closely with a dedicated team of healthcare professionals who share your passion for quality care and innovation. Key Responsibilities: Evaluate, plan, and implement personalized therapy programs tailored to residents’ needs and goals Perform initial and ongoing assessments to measure progress and adjust treatment plans Document all evaluations and treatments using Net Health EMR for efficient and accurate record-keeping Collaborate with nursing staff, occupational therapists, speech therapists, and other interdisciplinary team members Provide education to residents, families, and caregivers to support continued success beyond therapy sessions Maintain compliance with all state, federal, and facility policies and procedures What We’re Looking For: Current Pennsylvania Physical Therapist (PT) license in good standing (required) Strong communication, organization, and interpersonal skills Compassionate, team-oriented approach with a genuine desire to improve lives Experience in a skilled nursing or rehab setting preferred — but new grads are encouraged to apply!
