Primary Care Physician

We are seeking a compassionate and dedicated Primary Care Physician to deliver high-quality, value-based care to an established panel of predominantly geriatric patients. This role emphasizes preventive medicine, chronic disease management, and building meaningful, long-term patient relationships in a supportive outpatient environment. You will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations. Practice Info * 100% Outpatient (Geriatrics-focused position) * Collaborative, multidisciplinary care model * Physician-led, team-oriented environment focused on quality over volume * Strong emphasis on preventive care and population health management * Robust clinical support allowing you to focus on patient care Responsibilities * Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions * Deliver preventive care, wellness visits, and health screenings with a focus on early intervention * Develop and manage individualized care plans for a predominantly geriatric population * Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care * Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes * Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement * Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices Compensation * Competitive base salary + production-based bonus * Sign-on bonus and relocation assistance (based on qualifications) * Salary: 280000 Benefits * Fully funded malpractice insurance * CME allowance and licensing renewal support * 401(k) with employer match Shift & Schedule * Monday-Friday, 8:00 AM - 5:00 PM * No weekends or hospital rounds Requirements * MD or DO from an accredited medical school * Internal Medicine or Family Medicine or Geriatric Medicine * Active, FL license * Current DEA registration * At least 3 years of experience in Outpatient Primary Care * Strong clinical judgment with a focus on preventive and longitudinal care * Excellent interpersonal and communication skills * Commitment to a team-based, patient-first care model
