Location: Chambersburg, PA Keystone Health is seeking LPNs to join the team. We are a fast paced, expanding Community Health Center. Perform functions within the scope of a Licensed Practical Nurse (LPN); maintains competency to function as an LPN. Reviews and prepares Electronic Health Record (EHR) information prior to patient’s appointment. Prepare patient information for appointments by obtaining appropriate vital signs, reconciling medications, processing requests for medication renewals in a timely manner, and documenting accurately and timely in the EHR. Assist providers with providing care and performing procedures. Carries out provider’s orders, including completing requisitions for tests, medication administration and CLIA waived lab tests. Provides assistance to patients; communicates test and lab results as well as treatment ordered. Collects samples, such as urine, stool, sputum, and blood from patients for testing. Responsible for room prep between patients and at end of day. Orients and mentor’s new hires and students, as assigned. Attend department meetings and complete all the required training. Participate in organizational quality initiatives as requested. Understand various KH programs and communicate relevant information to patients. Assists with various clerical tasks as necessary.
