Job Description\n TalentBurst, Inc is seeking a travel Physical Therapist for a travel job in Battle Creek, Michigan.Job Description & Requirements\n \n Specialty: Physical Therapist\n Discipline: Therapy\n Start Date: 09/28/2026\n Duration: 13 weeks\n 40 hours per week\n Shift: 8 hours, days\n Employment Type: Travel\n \n \n Title:Home Health Physical TherapistLocation:Battle Creek MIDuration: 13 Weeks /OFFERING UP TO 26 WEEKS, NOTATE PREFERENCE IN SUBMITTALShift: Day (5x8 Hours) 8:00 17:00**Start Date- Oct 5th**\n HOME HEALTH EXPERIENCE REQUIRED\n Shift: 8:00am - 5:00pm\n Hours: 40 hours/FT\n Guaranteed Hours per Week: FT\n Call Requirement: No\n Weekend Requirement: Weekend rotation (every 4-6 weeks)\n Float Requirement: NoCoverage area: Van Buren, Calhoun and Kalamazoo County\n Leader will call to interview\n MI license required\n BLS (AHA) required\n Must have home health experience\n Current active and in good standing driver's license required, availability of car for business use on a daily basis, and proof of insurance required for Bronson at Home\n Evaluates plans and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to improve/restore function, increase strength, relieve pain, and enhance functional independence. Employees providing direct patient care must demonstrate competencies specific to the population served.Clinical Duties\n \n Interpretation and implementation of referrals; evaluates patient functional abilities/disabilities such as range of motion, strength, coordination, etc. through standardized assessment and testing; establishes functional baseline to be used throughout treatment as a gauge of progress and physical improvements\n Interdisciplinary discussion/rounds/clinics\n Establish education and treatment planning\n Family and community education\n Interdisciplinary team discussions hospital rounds/clinics\n Patient care goal setting and estimating length of treatment\n Direct patient care, including conducting individual and/or group Physical Therapy activities according to treatment plan; facilitates and instructs patients in the skills necessary to successfully participate\n May design and fabricate splints and other assistive devices\n Program development and participation in department process improvement\n Serving as a resource person in areas of expertise\n Providing in-services in areas of expertise for the Bronson Rehabilitation System\n May need to cross train and provide coverage at other departments within Rehabilitation Services as needed.\n \n Administration Duties\n \n Documenting evaluations, patient treatment plans, treatments performed, and progress; indicates condition in relationship to establish goals and alterations to treatment plan to enhance therapy potential.\n Documenting in the electronic health record per compliance timelines\n Responsible for communicating to leadership any limitations that prevent timely documentation.\n For each person served the therapist maintains accurate, legible and complete records.\n \n \n #PHYTH1\n \n JobDiva # 26-29546 (M)\n \n \n About TalentBurst, Inc\n \n TalentBurst Health & Life Sciences is an established provider of healthcare workforce solutions, servicing healthcare facilities across the United States for over 15 years.
