Home Health Aide

Home Health AidWe are looking for a Home Health Aid to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy, safe environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.ResponsibilitiesAssist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobilesCare for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal careMaintain records of patient care, condition, progress, or problems to reportBuild relationships with patients by conversing with them to help keep them mentally healthy and alertOversee medication management and assist as neededMust pass a Level 2 Background Screening through ACHA (Provided by company during onboarding if hired for the job.)Graduated from an accredited Home Health Aide program (CNA or HHA)High School Diploma or GEDOne-year prior professional experienceDriver's license requiredCPR/First Aide certification requiredCNA PreferredHIV/AIDS Training Certification Preferred, but not requiredProof of a negative TB test/health screeningCPR and TB Testing certification required if you do not have CPR or TB have to be willing to sign up through the company for credentialingCompensation: $15.00 - $17.00 per hour Heart2Heart Home Healthcare, Inc. provides professional, compassionate support services by certified nursing assistants which allow individuals we serve to live safely in their homes.
