Job Overview As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care to help patients regain independence in the comfort of their homes. Responsibilities Provide skilled nursing care to patients at their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family and healthcare team. Educate patients and families on disease management, treatment options and self‑care techniques. Maintain accurate records of patient care and documentation of visits, interventions and outcomes. Communicate promptly with patients, physicians, the clinical manager and other team members regarding patient or family needs and status changes; participate in care coordination and discharge planning. Required Qualifications Current nursing license in the practicing state. Valid driver’s license, auto insurance and reliable transportation. Current CPR certification. Two years experience as an LPN/LVN in a clinical setting. Preferred Qualifications Experience in a Home Health or Hospice setting. Pay and Hours Pay per visit: $33.00 – $47.00. Base salary: $53,000 – $73,000 per year. Scheduled weekly hours: 40. Additional Information TB Statement: This role is patient facing and part of Humana’s TB screening program; TB screening is required. Driving Statement: Requires a valid state driver’s license, personal vehicle liability insurance in line with state minimums.
