POSITION SUMMARY

The primary functions of this position are to coordinate care and provide mental health services for pregnant and post-partum mothers in the TGC Perinatal Mental Health Program called Moms & Babies First.


POSITION DUTIES

• Facilitates meetings of and coordinates care with the treatment team including members, guardians, family, natural supports, TGC Birth to Five, community agency stakeholders, justice system partners, and other service providers for mother and infant.
• Facilitates meetings of and coordinates care with the interdisciplinary treatment team including members, guardians, family, natural supports, community agency stakeholders, justice system partners, and other service providers for mother and infant.
• Creates treatment documentation: individualized, interdisciplinary care plans (AKA treatment or service plans); crisis prevention/safety plans; assessments (updates, crisis, substance use); internal and external referrals; treatment consents; residential treatment admission packets; discharge plans; Releases of Information; outside provider coordination of care communication documents; case presentations; etc.
• Creates and updates SHIFT (Safety, Healthy Infants and Families Thrive) Prenatal Family Care Plan or Plan of Safe Care with the mother for those members referred by Juvenile Court System SHIFT program.
• Assists members and families in obtaining needed services and community resources that help meet basic needs, including prenatal and postpartum visits, and infant well-child visits.
• Communicates and coordinates client care with family members, behavioral and medical health providers and community resources.
• Monitors and analyzes progress of members and families to determine needed interventions and responds to the needs identified including maintaining TGC Maternal Mental Health Registry, and collecting outcome tools like urine drug screens, PHQ9, GAD7, Edinburgh Postnatal Depression Scale and Arizona 4P's Plus Screen for Substance Abuse in Pregnancy.
• Provides specific services identified as needed through the care plan process, including case management, assessment, crisis de-escalation, life skills training, health promotion, vocational services, peer support, family support, group psychoeducation, etc.
• Provides effective, professional, trauma-informed, and recovery-oriented customer service to internal and external stakeholders.
• Recognizes, prevents, and responds to high-risk or dangerous situations, including knowing when to involve supervisors and/or medical practitioners during a crisis event.
• Demonstrates ability to recognize, prevent and respond to evidence of member injury, illness, abuse, neglect, and exploitation.
• Performs other duties as assigned.

DEPARTMENT SPECIFIC DUTIES

• May serve members and families residing on sovereign tribal nations, requiring travel.
• May complete required documentation for the SMI determination process.

GENERAL EXPECTATIONS

• Complete all TGC required trainings on time.

• Demonstrate flexibility and a willingness to learn.
• This position is 100% onsite/TGC campus, in the state of Arizona.

EDUCATION/EXPERIENCE

Required:

• Any one of the following education/experience combinations equivalent to four (4) years:
  • Bachelor's degree in behavioral health or a related field or discipline.
• Associate's degree and at least two years of health services experience.
• High School Diploma or Graduate Equivalency Diploma AND four (4) years of health services experience.
• Current driver's license and able to pass MVR screening.
• Must be at least 21 years of age.
• Ability to travel to other clinic sites, members' homes or the community if needed.
• Knowledge of available community resources for pregnant women and infants.
• Able to work autonomously or as part of a team.
• Must be able to obtain and maintain an Arizona Level One Fingerprint Clearance Card.
• Ability to develop expertise from additional training in perinatal mental health, parenting and infant wellbeing.

The physical demands of this position warrant:

• Sedentary Work: Occasionally lifting 10 pounds or less, sitting most of the time and may involve walking or standing for brief periods of time.
